Dear Sir/Madam,
We are open to any opinions, complaints and applications from You.

The above documents can be given in personnally or in writing directly to the managers or supervisors from the given department or in the Secretariate of the Director.

We reply in writing to the complaints given in in writing and the oral complaints are considered at the moment of making the complaint. 

The director of the Hospital in Śrem admits complaints and applications each Tuesday and Thursday at 11.00 - 12.00.
